This class Advance Directives and End of Life Decisions is open only to FCM members and is part of our community initiative to support our members in this area.

Have you ever wondered what would happen if you had a serious accident or illness and were unable to speak for yourself? Would your family know what medical interventions you wanted? Would they be able to ensure that your wishes are met?

Arwyn Elden, from Empath Choices For Care, will offer a class on advance care planning for FCM members.  You will be given the opportunity to explore your personal values and preferences about how you would like to be cared for if you were unable to care for yourself. This valuable tool can give your family the confidence and peace of mind they need to carry out your wishes.

In order to facilitate this process, participants will:

• Assess values to determine needs at the end of life
• Learn how to initiate essential conversations about quality of life and goals of care in regard to medical decision-making
• Understand how and why we may want to choose a health-care surrogate
• Review how to complete and utilize a living will
